Description

Micronase is used as an adjunct to proper dietary management, exercise, and weight reduction to lower the blood sugar levels in patients with type 2 diabetes mellitus (non-insulin-dependent diabetes) who are not able to manage their blood sugar. Glibenclamide (also known as Glyburide), the active ingredient present in Micronase tablets, is an oral anti-diabetic preparation, classed as a ‘second-generation’ sulphonylurea, with a potent hypoglycemic effect.

This medicine helps with the maintenance of blood sugar levels by stimulating the pancreas to increases insulin secretion, enhancing the sensitivity of tissues to insulin and reducing the glycogenolysis in the liver. In the treatment of type 2 diabetes, Micronase can be used in combination with insulin, Metformin or other anti-diabetic drugs. Its administration may also be considered (for off-label use) in pregnant women with gestational diabetes whose glucose level is not satisfactory despite following a diet and being physically active.

Indications

Treatment of type 2 diabetes mellitus in cases where the blood sugar level is not regulated by an adequate diet, exercise or weight loss alone.

Micronase is also used off-label for the treatment of pregnant women with gestational diabetes mellitus and whose exercise and dietary regimen cannot maintain blood sugar at a normal level.

MOA (mechanism of action)

Glibenclamide is a fast-acting oral antihyperglycemic medication of a sulfonylurea drug class. It is an insulin secretagogue that controls blood glucose (sugar) level mostly by acting directly on the pancreatic beta cells and triggering insulin release. By binding to ATP-sensitive potassium channels, located on the pancreatic beta cell membrane, and suppressing them, Glibenclamide causes the depolarization of the cell membrane and the opening of voltage-dependent calcium channels. This results in an intracellular calcium influx to the beta cell and succeeding stimulation of insulin release through exocytosis. The effect of the active substance Glibenclamide also includes a decrease in basal hepatic glucose production and quantity of glycated hemoglobin in the bloodstream as well as the improvement of peripheral insulin action.

Dosage

The dose of Micronase used for the treatment of diabetics is governed by the results of blood and urine glucose tests and should be advised by a doctor depending on each patient’s condition. It is important not to leave out any meal when you are on Micronase tablets. You should take the medication with or immediately after breakfast or, if none is taken, a midday meal. Swallow the tablets whole. Do not chew, crush, or break them.

Daily medicine allotment is to be taken as a single dose with or after the first main meal of the day (usually breakfast). Diabetic patients on Micronase tablets should be monitored by a healthcare provider and undergo regular blood and urine glucose determinations to obtain the optimum effect. One should continue to take the medication for as long as recommended by a doctor, as it will help control blood sugar levels and prevent diabetes complications.

Missed dose

If you miss a dose you should take it as soon as you remember about your missing. If it is the time for the next dose you should continue your regular dosing schedule. Do not take a double dose to make up for a forgotten one.

Overdose

If an overdose occurs and you are feeling unwell, you should seek emergency medical attention or contact your healthcare provider immediately. If you are experiencing symptoms of hypoglycemia, you should raise your blood sugar right away by consuming oral glucose (e.g., a small bar of sugar cubes, sweet juice, sweetened tea).

Storage

Store at room temperature under 25°C (77°F). away from excess moisture and heat. Throw away any unused medicine after the expiration date. Keep out of the reach of children.

Contraindications

Do not take Micronase if you are hypersensitive to Glibenclamide, any drug’s components, or other sulphonylureas.

Do not use the medication if you have type 1 (insulin-dependent) diabetes or your diabetes is complicated by ketosis.

Do not take the tablets if you are in a diabetic pre-coma or coma (a life-threatening diabetes complication that causes unconsciousness).

Do not use the medicine if you suffer from serious renal or liver impairment.

Do not take Micronase if you have severe thyroid or adrenal gland problems.

Do not use the medication if you have a glucose-6-phosphate dehydrogenase deficiency (G6PDD) (a hereditary condition in which red blood cells break down when the body is exposed to certain foods or drugs).

Do not take the drug if you have porphyria (an inborn disease of blood proteins affecting the skin, gut, and nervous system).

Exercise caution when taking the medicine in case you have serious cardiovascular problems.

Do not use Micronase tablets in combination with some medicines used to reduce pain and inflammation (like aspirin, azapropazone, phenylbutazone etc.), drugs to treat bacterial infections (e.g., isoniazid, co-trimoxazole, rifamycins, chloramphenicol, sulphonamides, and tetracyclines), monoamine oxidase inhibitors (MAOIs) used to treat depression, medications to treat fungal infections (miconazole) and gout (e.g., allopurinol, sulfinpyrazone, and probenecid), medicines used to treat high blood pressure (like bosentan, enalapril, diazoxide, captopril, and beta-blockers), thyroid hormone for the treatment of an underactive thyroid gland, drugs used to treat malaria (e.g. quinine and quinidine) and to thin the blood and prevent clotting (e.g., warfarin and disopyramide), medicines for stomach ulcers (cimetidine and ranitidine), medications for the treatment of cancer (e.g., crisantaspase), medicines used to treat psychosis and other mental illness (e.g., chlorpromazine), loop and thiazide diuretics to treat water retention and heart failure, hormone treatments and contraceptives (such as testosterone, oestrogens, corticosteroids and ocreotide, progesterones, oral contraceptives, anabolic steroids), ciclosporin for the prevention of an organ transplant rejection.

The medicine is not recommended for use if you are under unusual stress such as surgery, severe infection or trauma.

Do not consume alcohol while on Micronase tablets, it may affect the way the medication works.

Avoid driving or operating machinery while taking the medicine, as it may alter your vision and/or concentration ability.

Do not take the tablets if you are pregnant or breastfeeding without your doctor’s advice.

Micronase is not for use in patients under 18 years of age.
